Submitting your Blood Pressure Monitor Readings

We’ve made it easy for you to share your blood pressure readings with us, ensuring that your health is monitored effectively. Follow these steps:

1. Check your messages: You should have received an SMS (text) message with a link to submit your blood pressure readings. If you haven’t received it, no worries—keep reading.

2. Can’t find the message?  Click this link to request an online form. Select “Admin Query”    -> “Something Else.” Type a message requesting the online form, and we’ll send one across.

3. If you can’t access the internet. You can pick up a paper form from our Reception and you can then hand this form back in once completed
